Heart & Soul Free Clinic Now Offering Sports Physicals for Students in 6th through 12th Grades

Heart & Soul Free Clinic is now offering sports physicals for students in 6th through 12th grades who are uninsured, underinsured or are Medicaid recipients.
The clinic is now open on Wednesday evenings from 6 to 8 p.m. by appointment. To ensure a doctor or nurse practitioner will be available, appointments should be made by 5 p.m. the Tuesday evening prior to the clinic. If after 5 p.m. on a Tuesday evening, walk-in appointments may be available. Prospective patients should call for walk-in availability before coming to the clinic. Appointments may be made by calling 317-804-5782.
